We want to inform you that in August 2024 Deaconess is partnering with Fifth Third Bank on sending patient refunds with a variety of secure payment options.
If you are due a refund, you will receive an email containing a link to activate and access your funds facilitated by Fifth Third Bank. Please see the attached flyer showing different ways you can access those funds. If you have questions regarding this process, please contact Fifth Third Bank at 1-888-219-1370.
Rest assured, if you do not respond to the email within 30 days, a paper check will be mailed to you.
