Copy of College Transcripts
Verification of Nursing or PA license
Documentation of national certification
Current BLS: need to have 90 days from date of employment
Copy of DEA registration
Verification of prescriptive authority
Delineation form requesting credentialing and any special privileging
24 months of procedure/activity logs
Deaconess primary source verifiers will run a background check
Three references: one from a supervisor, one from a physician, two with same title as applicant
