Patient Information

We understand that you may feel anxious about your visit to a heart specialist. We will do our best to help you be prepared, and to answer any questions you may have. We want you to feel as comfortable as possible.
 
To be prepared for your appointment
  • Please arrive 15 minutes prior to your appointment for registration
  • Be sure to follow all instructions given if you are scheduled for testing
  • Items to bring with you to every visit
    • All bottles of your medication, including any vitamins and herbal supplements you may take
    • Your insurance cards and/or Medicare card
    • Driver’s license or ID
    • Copay payment
    • Any medical records you may have from another doctor or provider
What to expect during your appointment
Upon arrival for your appointment, please check in with the receptionist to complete the registration process. We know your time is valuable and make every effort to keep appointments as close to the scheduled time as possible. However, in healthcare, emergencies sometimes arise. On the rare occasion that your appointment is delayed, we will keep you informed about the delay.

When it is time for your appointment, one of our medical office assistants will lead you to an exam room. Every visit will include a measurement of height, weight, and vital signs. The medical office assistant will also review your current medications that you brought with you. You will then see the provider, who will interview you about your medical history and current condition. After the provider completes the exam, he/she will discuss his findings and recommendations with you. A registered nurse will provide further education as needed and review the provider's recommendations.

If additional testing is required, an appointment will be made at check out before leaving the office. If the appointment is unable to be scheduled at that time, someone from the Deaconess Heart Group will contact you to schedule the appointment.

If your family doctor referred you to our office, the physician will communicate with him or her to report  findings and recommendations.

How to cancel your appointment
Should you need to cancel your appointment for any reason, we request that you contact us 72 business hours prior to your appointment. Patients with multiple missed appointments can be assessed a no-show/cancellation fee, and their account may be charged according to our guidelines.

When patients cancel too close to their scheduled times, or simply don't show up at all, prevents us from using that appointment time for other patients requiring our care. Cancelled appointments also impact the continued high quality care that we want to provide to our patients. In the event that a cancellation is necessary, please contact our office at 1-800-432-7868 or 812-464-9133 to cancel/reschedule your appointment.

Established patients are also able to send a message requesting a change in their appointment via My Chart.
