COVID-19 Vaccine Scheduling

How Vaccines Will Be Administered at Deaconess
Deaconess will be following guidelines and protocols set by state departments of health/state government. Healthcare workers continue to be the priority for receiving the vaccine.  

NEW INFORMATION:
  • Indiana residents age 55 and older can now register for COVID-19 vaccination. (effective 3/2/21).
  • Kentucky is vaccinating individuals age 60 and older who live or work in Kentucky (effective 2/19/21). 
  • Call List Registration is available below on this page, to be on a list to receive one of the extra doses from the vaccine clinics at the end of each day.
  • More people will be eligible in the coming weeks, pending direction by state health departments.

Indiana COVID-19 Vaccine Scheduling:
For healthcare workers, first responders, or those age 55 years or older, who live in Indiana.
 
Henderson County, KY COVID-19 Vaccine Scheduling:  
For healthcare workers, first responders, those age 60 years or older, childcare providers, or those who have contact with COVID-19 infectious material, who live or work in Kentucky. 
Union County, KY COVID-19 Vaccine Scheduling:   
For healthcare workers, first responders, those age 60 years or older, childcare providers, or those who have contact with COVID-19 infectious material, who live or work in Kentucky. 
 
 
Call List Registration

At the end of each day, there are sometimes a few vaccine doses left over, which must be used immediately.

Following recently-updated state guidelines, we are collecting contact information for people who would like to be on a call list to receive one of these extra vaccine doses.

Here are details:

In Indiana
    You must be age 18+ with at least one of the conditions listed below.

    The CDC has stated that adults with the following conditions are at increased risk for severe illness from COVID-19:
  • Cancer, undergoing treatment currently (chemotherapy and/or radiation)
  • Chronic kidney disease
  • COPD (chronic obstructive pulmonary disease)
  • Down Syndrome  (age 16+ eligible; choose age 18 in form)
  • Heart conditions, including heart failure, coronary artery disease, or cardiomyopathies
  • Immunocompromised state from organ transplant
  • Obesity (BMI of 30 or higher)   (BMI calculator)
  • Pregnancy
  • Sickle cell disease (age 16+ eligible; choose age 18 in form)
  • Smoking
  • Type 2 diabetes
In Kentucky
    You must be age 55-59.  

For all registrants
  • You must be willing to come to get your shot at very short notice, during afternoon or evening hours, and be able to arrive within 45 minutes of receiving the call.
  • You must answer your phone; if you do not answer, staff will move on to the next name on the list.
  • Entering your information on this form is not a substitute for making an actual appointment at a later date, when eligible.
