Register online by filling out our secure online form. >>Pre-Registration Form Call our automated pre-registration system. Using your touch-tone telephone, you can answer the questions necessary to register for your visit. Automated Pre-Registration: 877/444-6502 Call our pre-registration telephone line to pre-register with a Deaconess pre-registration specialist. Available from 8 am to 6:30 pm, Monday through Friday.

We understand that your time is valuable. To reduce your time in the registration process at the hospital, please pre-register using the online form.

• To pre-register, you may complete our secure online pre-registration form.
• This online form is for Deaconess Hospital and Gateway Hospital only. If you are wanting to pre-register at The Women's Hospital, please use The Women's Hospital's registration form.
• You may pre-register online at any time of the day. You are welcome to call the Patient Registration department or a Pre-Registration associate to answer any questions.
• If your information is incomplete or needs clarification, the staff may need to complete the registration by telephone or when you arrive.
• You will need to complete the registration process at the Patient Registration area upon your arrival at the hospital.
